Parents,

The checkout policy for students requires a parent or guardian to sign out his/her child in the check-in/out office.  However, for the mid-term test schedule posted below, a different procedure will be used.

If you grant your child permission to leave after testing is complete at 11:50 a.m. please complete the form below and have your child return it to his/her first period teacher on or before Monday, December 13^th.   The student must have his/her own transportation and have your permission to leave. Lunch, afternoon study hall, and bus transportation will be provided for all students.

Please keep in mind that December 16^th, 17^th, 20^th, and 21^th are important test dates for our students. Please make every effort to schedule appointments during the afternoon or on another date. Also, we will not be able to accommodate telephone checkouts and classrooms will not be called during designated test times.
